RESUMO

INTRODUCTION: Numerous international studies have reported on the relevance of music therapy for aphasia, but music-based therapy for the rehabilitation of acquired language and speech disorders is not common in the clinical practice of Hungary. OBJECTIVE: The aim of our study is to provide insights into the composition of professional teams involved in aphasia care in active neurology and stroke wards and rehabilitation departments in Hungarian hospitals, with a special focus on the presence of music therapists. We are looking for the answer to why the employment of music therapists in hospitals is so low in our country. METHOD: For our research, we selected the relevant institutions and departments from a list of hospitals on the website of the National Directorate General for Hospitals. Data were then collected from the websites of the hospital departments, supplemented or clarified with information from the head of department's physicians where necessary. RESULTS: None of the active neurology and stroke wards employ a music therapist. Two of the rehabilitation wards have a total of 4 music therapists. DISCUSSION: The low number of trained professionals in music therapy for aphasia is due to financial reasons, lack of professionals and lack of professional demand. CONCLUSION: Our research shows that music therapy is deeply underrepresented in the field of aphasia rehabilitation in Hungarian hospitals. The reasons for this are quite diverse and their elimination would require effective intervention in many areas. Orv Hetil. 2023; 164(19): 747-752.

RESUMO

BACKGROUND: People with disadvantages are a high-risk group of unemployment or underemployment. Disadvantages include disability, under-education, or being a member of a minority, etc. Effective labor market programs could be a key in raising employment and quality of life among this high-risk group of society. The TOP 6.8.2.-15-NA1 project is one of the main Hungarian labor market programs. The project's primary aims are increasing the employability of disadvantaged unemployed and supporting the efficiency of job-seeking. METHODS: Our goal was to analyze the effects and methodology of the TOP 6.8.2.-15-NA1 project in Hungary. The sample of our study contains participants of the project (n = 300), based in Zala County, Hungary. RESULTS: After 28 days, 53.3% of participants had a job. At the 180th day status, the rate of employed people was 47.3%. We could identify low-educated participants and older participants as higher-risk groups of long-term unemployment. CONCLUSIONS: We emphasize the role of these services (job-seeking clubs, organization of job fairs, and mentorship) in the long-term individual success of participants. Improving the employment rate for people with disadvantages is a critical factor for enhancing the quality of life for individuals with disadvantages.

RESUMO

Összefoglaló. Bevezetés: A férfiakat érinto egyik leggyakoribb malignus ráktípus a prosztatarák. A Nemzeti Rákregiszter adatai alapján hazánkban évente több mint 4600 új eset kerül diagnosztizálásra. Célkituzés: A rákbetegek pszichoszociális háttere napjainkig alulvizsgált terület Magyarországon. A szerzok célja a magyar prosztatarákos férfiak szociodemográfiai, egészségi, mentális állapotának vizsgálata volt. Módszer: A vizsgálatban 100 prosztatarákos férfi vett részt, akiknek más urológiai betegsége nem volt, továbbá más daganatos betegségbol kifolyó kezelésben nem részesültek a kérdoív kitöltésekor. A részvétel anonim és önkéntes volt. A standard validált tesztek mellett - mint a Beck Depresszió Kérdoív, Rosenberg Önértékelés Skála - szociodemográfiai, egészségmagatartás- és életmód-kérdéscsoportokat tartalmazott kérdoívünk. Statisztikai analízis: Leíró statisztikát, khi-négyzet-próbát, lineáris regresszió analízist, Fisher-egzakt tesztet, kétmintás t-próbát végeztünk 95%-os valószínuségi szinten az SPSS 20.0 és a Microsoft Excel 2016-os programban. Eredmények: Az egészségi állapotot a lakhely településcsoportja (p = 0,024), az anyagi helyzet (p = 0,001), a krónikus betegség (p = 0,000), a fizikai aktivitás (p = 0,000) és az alkoholfogyasztás (p = 0,001) befolyásolta. A társas támogatás (p = 0,726) ellenben nem bizonyult befolyásoló tényezonek. A megkérdezettek 66%-a volt elhízott a BMI alapján. Helyesen csupán a megkérdezettek egyharmada táplálkozott a ma érvényes MDOSZ-ajánlás alapján. 62%-uk enyhén depressziósnak volt mondható a Beck Depresszió Kérdoív alapján, 73%-uk önértékelése azonban a Rosenberg Önértékelés Skála alapján átlagosnak volt mondható. Orv Hetil. 2021; 162(10): 383-391. INTRODUCTION: Prostate cancer is one of the most common malignant cancers amongst men. More than 4600 men are diagnosed with prostate cancer every year in Hungary. OBJECTIVE: The psychosocial background of cancer patients is an underexamined area in Hungary. The aim of the authors was to obtain information on the sociodemographic status, health and mental status of Hungarian men with prostate cancer. METHOD: 100 prostate cancer patiens were included in the study who did not have any urologic diseases and did not go under any therapy due to any other type of cancer. The applied questionnaire was put together by the authors about sociodemographic and health status, lifestyle. The Beck's Depression Inventory and the Rosenberg Self-Esteem Scale were also used. STATISTICAL ANALYSIS: We used descriptive statistics, chi-square test, linear regression, Fisher's exact test, independent t-sample test on a 95% probability level in the SPSS 20.0 and Microsoft Excel 2016 softwers. RESULTS: The settlement classification (p = 0.024), financial status (p = 0.001), chronic illnesses (p = 0.000), physical activity (p = 0.000) and alcohol consumption (p = 0.001) affected the health status. Only one third of the respondents ate healthy according to the recommendations of the MDOSZ. 66% of the respondents were overweight. 62% of the respondents were depressed according to the Beck's Depression Inventory, however, 73% of them had average self-esteem measured by the Rosenberg Scale. We proved that with age either the depression score or the self-esteem score did not increase. CONCLUSION: Our results roughly mirror the national and international literature about health behavior. No connection was found between self-esteem and depression. Orv Hetil. 2021; 162(10): 383-391.

RESUMO

The prevalence of the metabolic syndrome, type 2 diabetes mellitus, cardiovascular diseases, obesity and depression have increased during the recent years. As the sexual dysfunction is also frequent, we aimed to search for the associations between sexual dysfunction and the metabolic syndrome and its components, respectively, by reviewing the literature. The clinical and biochemical components of the metabolic syndrome included cardiovascular disease, type 2 diabetes mellitus, visceral obesity and depression, furthermore, insulin resistance, atherogenic lipid profile, hypogonadism, chronic systemic inflammation and endothelial dysfunction were all demonstrated to affect adversely the sexual function. The dysfunction of the sexual arousal response shows a strong association in men and a milder one in women with the cardiovascular diseases and depression. Sexual function in diabetes mellitus is mostly impaired by microvascular injury, polyneuropathy and autonomic neuropathy. Erectile dysfunction and disorder of the female sexual arousal response and the orgasm, respectively, are associated with insulin resistance, atherogenic lipid profile and systemic inflammatory condition in overweight or obese patients. Sexual dysfunction particularly in men can be an early sign of the severe complications of metabolic syndrome. The pathogenetic link between the metabolic syndrome and the sexual dysfunction seems to be the insulin resistance. Both metabolic syndrome and sexual dysfunction can be restored by altering the lifestyle. Orv Hetil. 2019; 160(3): 98-103.

RESUMO

INTRODUCTION: Congenital structural clubfoot is the most common congenital deformity of the foot. Despite successful treatment, the affected side may present signs of athrophy, differences in length, functional maldevelopment and in some cases it may also result in pain. Although several international studies have been investigating the quality of life of individuals with clubfoot, no research has been done in Hungary. Patient-based assessment is used for the evaluation of the effectiveness of treatment and for the comparison of the traditional versus the Ponseti method. AIM: To gain permission for the use of the internationally most well-known Clubfoot Disease-Specific Instrument (hereafter: 'Clubfoot Specific Questionnaire') consisting of 10 items and two subscales, and furthermore, to adapt its parent and child-version for Hungary and to carry out the internal and external validity testing of the measure. METHOD: The Questionnaire was completed by 75 individuals (44 boys, 28 girls) belonging to three age groups (under age 12, aged 12-17.99 years and those aged above 18); the mean age was 12.45 years (range: 2-44 years). The measures used: the Hungarian version of the 'Clubfoot Specific Questionnaire' (adapted versions for the given age groups), and the Hungarian version of EQ-5D-3L to measure the general quality of life. RESULTS: Both the parent- and the child-version of the 'Clubfoot Specific Questionnaire' showed adequate internal validity (parent-version: 0.872, child-version: 0.923); the individual items showed medium to strong correlations with the EQ-5D-3L dimensions besides the existing significant connections. CONCLUSION: The Hungarian adaptation of the measure proved to be successful. The validated questionnaire provides a further tool for the assessment of the quality of life of individuals with clubfoot and also for the evaluation of the success rate of therapies. RESUMO

INTRODUCTION AND AIM: Employment of mindfulness-based programs and techniques in the treatment of cancer patients have spread exponentially in the past decade. A large number of studies have proved the effectiveness of the program in the reduction of stress symptoms, depression and anxiety, and the improvement of sleep and mood patterns. The present study was done at Firebird Foundation and examines the effectiveness of the complex, MBCR (mindfulness-based cancer recovery)-based psycho-social intervention in the treatment of cancer patients. METHOD: The experimental group consisted of 101 people with various types and states of cancer. The control group consisted of 72 adjusted people. We did measurements (PPS, HADS, FACT-G, POMS-SF, Freiburg Adaptation Questionnaire) before the intervention (T1), right after the 8-week intervention (T2), and 6 months after the intervention (T3). RESULTS: The experimental group showed significant improvement in the measured psychological variables, and in few aspects of life quality between T1 and T2. There was a decrease in levels of observed stress and depression, and an escalation in optimism and vitality. We found a decrease in depressive and minimizing coping scores as well. There was a significant improvement in two dimensions of quality of life, and all these positive changes remained present by the end of the follow-up period (T3). In the control group, we only found a significant increase in minimizing coping scores at the time of T3 measurements. This is the first impact assessment study done on cancer patients using MBCR techniques in Hungary. CONCLUSIONS: The MBCR program is a very effective psychosocial intervention among cancer patients; it has a long term significant impact on mood and quality of life.
